Please complete the following information. Certain fields are required and this form cannot be submitted without them. Incomplete or inacurate information may delay adoption approval.

I understand my services are provided in a voluntary capacity and agree to provide services to Paws For Life as a volunteer. *
|
|
|
I understand I will receive no compensation, salary, employee benefits, or payment for the services I render. *
|
|
|
I understand Paws For Life handles large numbers of animals on a daily basis. The temperament of these animals is often unknown to Paws For Life staff. *
|
|
|
I agree to hold Paws For Life harmless for any injury(s), which I might sustain from handling animals during the course of my volunteer duties.
|
|
|
I understand and agree to assume all risks involved in duties I perform for Paws For Life (Such duties might include, but are not limited to, animal handling, custodial work, kennel assistance and other volunteer duties). *

|
|
I agree to familiarize myself with the Paws For Life policies and procedures and will fully comply with both the letter and spirit of these policies and procedures. *

|
|
I understand Paws For Life expects high standards of moral and ethical treatment of animals under its care. *
|
|
|
I agree to never strike an animal, nor handle or treat an animal in such a way it would be construed as rough or abusive. I will always exercise compassion and care with animals. *
|
|
|
I agree not to represent Paws For Life outside my immediate volunteer capacity. Paws For Life employs designated spokesperson(s) to handle concerns of animal welfare issues within and outside the Center. *

|
|
I agree to hold confidential all information that I may obtain, directly or indirectly, concerning clients, animals and staff.
|
|
|
I understand failure to comply with any of this agreement, or for any reason whatsoever, Paws For Life, at its sole discretion, may immediately terminate my volunteer services. *
|
|
|
I agree to accept a position in a voluntary capacity with Paws For Life Animal League. Actions and services are rendered to Paws For Life with generous and charitable motives. Paws For Life will accept no liability for voluntary services. *
